How to Dispute a Report

If you are the subject of a report and disagree with either the factual accuracy of the report or whether the report was submitted in accordance with NPDB reporting requirements, you can dispute the report. Before disputing the report, you should contact the reporting organization listed in Section A of the report to resolve the issue.

Entering the report into dispute status does not trigger a review of the report. When a report is in dispute status, the reporting organization can correct the report, void the report, or choose to leave the report unchanged. If after 60 days you have received no response from the reporting organization, or you are unsatisfied with the response you received, you can elevate the report to dispute resolution.

When you dispute a report, you may also add a statement to provide any additional information you would like included with the report. The statement can be added separately without disputing the report. The statement and dispute are disclosed to the reporter, all querying organizations that have received a copy of the report during the past 3 years, and included in future query responses.

To Dispute a Report

Note: Only the subject of the report can sign in to their report.

  1. On the home page, select Sign In to View Your Report.
  2. On the Sign In To Review Your Report page, complete the information requested to sign in to your report.
    Note: If you are signing in to your report for the first time, use the report number and temporary access code on the notification letter. For security, the NPDB may require you to enter your Social Security number (SSN) or Individual Taxpayer Identification Number (ITIN) to sign in.
  3. On the Your Reports page, select Updates and Options.
  4. On the Options page, select Add/Update a Statement or Dispute to go to the Report Summary page.
  5. Find the Current Dispute Status section and select Dispute this Report. To remove the report from dispute status, select Not Disputed.
  6. Complete the Certification section and select Submit to the NPDB.
  7. On the Subject Statement and Dispute Status page, select View Report to view and save a PDF of the report.
